Have you ran a long SMART on that drive?

Can you confirm that all member disks are currently visible?

Failing all that you could try zpool import -FX NAS04_VOL01 and let ZFS scan for recoverable options. Perhaps open a tmux session first as it can take a while.

Yes, smart test on the failing drive shows errors but I can see all the disks

I think sdb is causing all the issues but it’s not fully dead yet. Personally I’d power down and remove sdb (check serial numbers correspond beforehand). Then power up and see if it either auto imports or if you can import it albeit in a degraded state. You have a Z2 so losing a drive shouldn’t be the end of the world.
