25.10.0.1 upgrade crashed the NAS - Resuscitated

Hello,

I do have two TrueNAS installations, (different mobo and CPU) but same TN versions installed.

On the first small NAS the installation of the 25.0.10.1 upgrade went smooth and the system rebooted normally.

On the second larger NAS the upgrade installation went successfully, but the system never returned on.

I got this error messages from console

that says one disk has issued (sdd) but since the GUI doesn’t load nor I can access the console, how do I can recover the system?

It’s hard to believe an upgrade can mess up a system, and even harder to believe a single disk down prevent booting or access the GUI.

After a forced reboot and a complete shutdown and restart the system returned accessible.

I was able to access the GUI, replace the failed drive and now the entire NAS seems working normally.

it’s still a mystery why a single drive failure took down the entire NAS.
