This 100% looks like what I wouldn’t want to see… that being said - you aren’t sure if the boot drive is connected to it, so I’ll withhold judgment until you can validate physically when you’re on site.
Try moving boot drive straight to motherboard if possible - if problems stop, get rid of that HBA. If problems persist, then HBA is likely working ok, and you should do a clean install on a replacement boot drive & upload your config.
