The drive device names can change as boot time. I am guessing the log was from a different time than the GUI screenshots. SDA next boot could be SDB or SDC
Either way, I would do SMART Long tests and look at the results.
the naming of the drives has been consistent since last hw change.

Do you have SMART Long tests run on your drives and checked the info?
Posting complete details on your hardware, OS version and how everything is connected may give a clue. All we have is two conflicting sets of screenshots.
Best to at least run a smartctl -a /dev/sd(drive letter goes here) whenever you see something like that in the logs & jot down the sn# & glance at the smart attributes. At least you’ll know beyond doubt that it is the same drive & can take action based on the sn# labelled on the drive.
…there have been multiple reports in the past where people have put their pool at risk because they assumed that the SDA/B/C/etc did not change & they pulled the wrong disk to swap. Better safe.
Speaking of which, mind providing the output of the command above? sudo as necessary.

This’ll translate the values for fields 1, 7, & 195 into something readable for humans. Not an issue, just a quirk of how Seagate drives present smart attributes.

I see nothing wrong in any of the attributes, only ‘problem’ is that only short tests have been run, never a long test. In theory something could be wrong & was missed.
However, I’m more curious now on how these drives are connected. Random IO errors that aren’t detected on smart logs are curious to say the last.
Any chance we can also get the smart output for the boot-pool drives? (if not seagate HDDs then you don’t need to run the additional parameters, just a smartctl -a /dev/sd(whatever) would be fine.
But do also let us know some hardware details; motherboard, how drives are connected (direct to motherboard, hba… something less kosher?), is truenas virtualized or running on bare metal, etc.
I could imagine this being as simple as needed to reseat some cables/replace a sata data wire based on the clean smart outputs thus far.
I thought the last two results in the DRIVES file above were the boot devices. Did you want Smart Long for those, Fleshmauler?
first, it’s been working fine for a long time.
mb: Asrock B550 Phantom Gamin ITX
cpu: AMD Ryzen 7 PRO 4750G
mem 32gb ECC - 3200mhz
boot-pool, 2x msata drives connected to the onboard sata controller
bigpool, 5x 8tb sg drives connected via m.2 sata adapter.
not ideal, i used a hba before, but i needed the single pci slot for a gpu.
but stil this setups has been running fine for months.
as a understand it, the mirror boot pool in truenas, it’s just a mirror, in case of a failure of the “active” boot drive, you still have to reboot to the “good” device right?
Oh - you’re right, I was only looking at the second file, but the boot drives are indeed in the original!
Ehh - possibly sketch. A lot the recommendations are for what’ll work least poorly when something goes wrong instead of what’ll simply be functional. I’m not blaming the hardware or setup, but it could be a factor & it is reasonable to review.
System would still run fine as long as 1 of the mirrored drives is working; both are active.
As far as rebooting with a failed mirror - depends on the kind of failure. For example; lets say sda & sba are mirrored boot drives. The bios uses sda to boot. sda suffers a bad failure, data is corrupted, it in no way is a functional boot drive, expect for the fact that the motherboard can still detect it & sees some sort of boot partition on it. The motherboard would still try to use sda to boot (the boot won’t be successful).
In this example, you’d need to either change the bios to use sdb, or disconnect sda.
The mirror’d boot drives feature is more for total drive failure (no longer detectable), or so that you can hotswap a failed/failing boot drive without shutting down the system (gotta get that 99.999% uptime). It is a lot of fun to pull a boot drive with the system still running & have a new one resilvered & have no downtime, but realistically not the most useful feature for a home user.
/end of theoretical example.
…Anyway, going back to the original question, @SmallBarky was right & the smart outputs are indeed present for the boot drives. In one of them I have nothing but unknown attribute; always useful… In the other there is an error:
Error -4 occurred at disk power-on lifetime: 0 hours (0 days + 0 hours)
When the command that caused the error occurred, the device was in an unknown state.
Lets go with basics for now; try reseating the connections to “KINGSTON SKC600MS512G Serial Number: 50026B77858E2966”. If possible, try a different sata data cable & a different port on the motherboard. It could be that simple.
If @winnielinnie was here, he’d tell you to run memtest for the next few days ![]()
Edit: typos, corrections, and readability edits. tldr; Barky was right. Mirrored boot are both active & system should still run without needing a reboot in case of single failure. Reseat/swap cables, move ports, retest. Ends with a joke.
