Are you sure you’re running 24.04?
I just find it odd that it says 6.12-something - I thought that was new in 25.04.

Maybe that “6.12” refers to something else and just happens to coincide with the relatively recent swap to the 6.12 kernel in the 25.04 nightly, which is alpha level software at this point, at best.

Either way, I don’t know why it’s failing, my first guess would be that if this is otherwise stable hardware, your boot device is faulty, or possibly the install media.
