Drives not working

I have several sas drives that over the last year stopped reading or show errors. I built a Windows based test mule to format and diagnose drive issues. All drives are formatted at 512. The ones that report 100% health ,performance and no bad sectors I reformatted and installed back into my nas. Some drives work with no issues, others continue with issues. Does anyone know any software, or have any idea what is causing drives to fail?

What are your current hardware, OS and pool details. In the past you had unconventional set ups?

My server setup uses an Asus p6T mobo w/ 32gb of ddr3 ram. the processor is an Intel Xeon E3-1240. With an LSI SAS9212-414E 4gig HBA. It has a 256 gig sata ssd boot drive. 2 sata 2TB rust drive and 4 3 TB Hitachi SAS drives.

My JBOD has currently 8 3TB SAS drives connected to an HP 468405-002 HBA. Each unit runs a 1000w power supply.

My testing rig runs windows 11 25h2 with a HP 468405-002 hba. I use software called Hard Disk Sentinel to test for drive errors, bad sectors, health, and performance

Current OS is Truenas Scale 25.10.3.1. I have 3 pools (1) is called Transcode it has 3, 3TB disks as raidz1. (2) is called Data also 3, 3TB disks set as raidz1. (3) is Apps setup as 2, 2TB disks set as mirror. 2 disks were tested and passed on my test rig but show 0B disk size. 2 more are empty but are available. 3 more do not show at all, but passed on my test rig.

Is there something wrong with my server limiting the amount of disks, or is there a better way to set up my test rig to prepare the disks for use?
